  • The following is required:
    • Government issued photo ID such as driver license(s) of you and your spouse, if applicable
    • Copy of social security cards for each individual included in tax return
    • Copy of last year Federal and State income tax returns, if available, including any amendments
    • Tax Forms and IRS Correspondence
      • Wages(W-2), interest(1099-INT), dividends(1099-(DIV), capital gains/losses, brokerage statements (1099-B), unemployment compensation(1099-G), pensions and other retirement income(1099-R), Social Security benefits (SSA-1099)
      • Self-employment income
      • Miscellaneous income reported on Form 1099-Misc, 1099-NEC
      • Other Income not reported elsewhere
      • Gambling Income (W-2G)
      • Itemized deductions – Mortgage Interest Statements, Real Estate Taxes Paid, Medical Expenses, Charitable Donations, Large purchases
      • Cancellation of nonbusiness credit card debt(1099-C)
      • IRA Contributions
      • Credit information such as College Tuition (1098-T), Child/Dependent Care information
      • Special forms if they apply to you
        • Stimulus payments received if any –
        • Advance Child Tax Credit Payments –Letter 6419 for 2021 returns
        • Health Insurance Marketplace Advance Payment- (1095-A)
        • Health Savings Account (1099-SA)
        • Student Loan Interest(1098-E)
        • Letter 6475 and Notice 1444-C for prior years returns
      • Other Information
        • Any other IRS forms/correspondence not mentioned above
        • Voided check if requesting direct deposit or direct payment
        • Estimated Tax Payments
